• Hotels-the plan benefit covers:

• Car Service-includes ground transportation to and from the airport, consultation appointment, surgery, and follow-up appointment only. Any other trips are the responsibility of the patient and do not qualify for reimbursement unless preapproved.
• Car Rental-includes reimbursement of car rental fees, toll fees (if applicable), parking, and gas.
• Personal Vehicle-includes reimbursement of mileage and toll fees (if applicable).

• The Plan covers $50 per day for both the patient and companion. It is reimbursed after surgery, and it is not necessary to provide meal receipts.
